R/M SCCHN who had progressed on or within 6 months after platinum-based therapy (Figure 1)

INDICACION NIVOLUMAB: Antes de iniciar el tratamiento, los médicos prescriptores deben tener en cuenta la aparición retardada del efecto de
nivolumab en pacientes con características de peor pronóstico y/o enfermedad agresiva. En el cáncer de cabeza y cuello, se ha observado un
mayor número de muertes durante los 3 primeros meses en el grupo de nivolumab comparado con el de docetaxel, siendo los factores asociados a
muertes tempranas el estado funcional (ECOG >0), la enfermedad de progresión rápida durante un tratamiento previo con platino y la alta carga
tumoral.

1. Enfermedad locorregional: estudios en marcha
2. Recurrente/M1 tras tto con platino-RT
• No candidatos a rescate IQ/IMRT
• No candidatos a retratamiento con platino
• No rápidos progresores
3. Tras progresión a primera línea con platino en combinación
4. Efecto abscopal: NO estrategia
CONCLUSIONES
- El tratamiento con anti-PD1 tiene beneficio en supervivencia en pacientes con cáncer de cabeza y cuello
- Evidencia nivel I tras progresión a platinos - La selección de pacientes candidatos es compleja (BMK)
- Los ensayos de primera línea y de combinación es posible que respondan cuestiones
actualmente abiertas y abran nuevas vías
