Actividad laboral no relacionada?
Varón de 37 años, trabaja en un taller metalúrgico
Junio 2003: presenta cuadro compatible con brucelosis
confirmado por serología. Recibe tratamiento adecuado.
Refiere ingesta de quesos caseros en el año 2001.
Mayo 2004: presenta síntomas de recaída. Se confirma recaída
por aislamiento en hemocultivos de Brucella Suis 1 y altos
niveles de aglutininas séricas.
Al reinterrogar recuerda haber reparado caños provenientes
de un frigorífico de cerdos tres semanas antes del primer
episodio
La Asociación Americana de Bancos de Sangre (American
Association of Blood Banking-AABB-),
Establece la obligatoriedad de efectuar a todo donante las siguientes pruebas:
• VDRL
• antígeno de superficie de hepatitis B (HBsAg)
• antígeno de core de hepatitis B (anti-HBcore),
• anticuerpos contra HIV-1, HIV-2
• hepatitis C • hepatitis C
• virus linfotrófico de células T humanas (HTLV I/II) .
• Lo anterior no excluye la utilización de exámenes adicionales para patologías de una alta incidencia en la
población, como es el caso de enfermedad de Chagas a nivel latinoamericano , ni la implementación de
nuevas tecnologías o marcadores que brinden mayor seguridad al producto sanguíneo, como la
detección de la proteína p24 del HIV

Neonatal brucellosis following blood transfusion has not previously been
reported. A premature male infant born at 24 weeks gestation
developed low grade fever and decreased activity and showed poor
weight gain at 45 weeks post-menstrual age. Blood culture grew
Brucella melitensis and the brucella antibody titer was positive. He Brucella melitensis and the brucella antibody titer was positive. He
received a 6-week course of septrin and rifampicin and made a full
recovery. The infant had received a blood transfusion 5 days prior to
his illness. The blood donor had symptoms suggestive of brucellosis,
and it was suspected that the blood transfusion was the source of
infection but this could not be confirmed as the donor was not
traceable. It is suggested that, in areas endemic for brucellosis,
prospective blood donors should be questioned about symptoms of
brucellosis, and serological tests to screen for brucellosis might be
indicated.
