En los primeros tiempos de la quera~oplastia la ate~ción del cirujano iha dirigida especia lmente a obtener la adherencia del injerto, siendo secundaria la mayor o menor transparencia de éste. Al perfeccionarse las técnicos quirúrgi ..
cas la adherencia del injerto dej6 de constituir una preocupación, siendo ésta
inc6gnita reemplazada por la de obtener uno buena tr~nsparencia.
Hoy dra solucionado satisfactoriamente este último punto, ha emp.ezado a aplicarse la queratoplastia cada vez en ojos en mejores condiciones, hasta el

punto que actualmente, puedo proponer la queratoplastia como interv.enci6n para modificar la refracción en ojos amétropes sin lesiones cornea les.
A las cuatro finalidades por 1", que
se practica una queratoplastia, pode~ mos añadir la finalidad refractiva.
Tenemos pues queratoplastias que se
practican con fin: Optico. Reconstructivo. Terapéutico. Estético. Refractivo.
Al practicar queratoplastias en cór~
neos opacas con finalidad 6ptica , nos ha sido dable observar intensos grados de miopía e'n los injertos que habían quedado un poco abombados e hipermetropías cua ndo en raz6n del proceso cicatricial, la córnea quedaba apIanada. Por ello en 1948 aconsejé practicar las queratoplastias ópticas en los ojos miopes empleando un injerto algo me~
nor que la trepanaci6n a fin de aumentar el radio de curvatura cornea !, y al dism inuir el poder dióptrico de la córnea , corregir o por lo menos disminuir el grado de a metropfa.
En esta forma intervine varios casos con resultados favorab les.
. Igua lmente en algunos enfermos que
presentaban opacidades de cristalino y que posteriormente a la queratoplastia debfan ser intervenidos de su catarata, coloqué un injerto algo mayor a fin de aumentar la curvatura corneal y corregir en parte la hipermetropía que resultada de la afoquia.
Estos pacient.es fueron intervenidos por la técnica penetrante por lo que, debido a l riesgo, que aunq ue pequeño siempre existe, no creí prudente sentar la indicoci6n de una queratoplostia en un ojo con córneo sano con el único fin de modificar su ametropía.
El exa men de los enfermos de quera~ toco no operados de q',eratoplast ia penetrante de 6 mm., en los que a pesar de obtener excelentes resultados vis ua~
les, persiste a veces una ametropía dis~ creta, debida en parte a que el injerto
.. queda c.o loca.d.a en .la . pu.nto_d.eJ cono, me indicó la conveniencia de practicar
queratoplostias de mayores dimensio
nes. a fin de normalizar completamente
la curvatura corneol. Castroviejo .en
uno de sus recientes trabajos, insiste es
pecialmente sobre el particular, y cita casos en que han precisado tres inter
venciones de grandes dimensiones hasta conseguir una normalizaci6n de la cur
vatura.
Por otra parte, el estudio de la hipotrofia del injerto. me indicó la conve
niencia de no practicar queratoplostios
penetrantes mayores de 6 '12 mm. y la necesidad de limitar el injerto con tejido corneal de espesor normal. Estas
cond iciones no podía n ser reunidas en
los casos de grandes querotoconos, por lo que dec idí regularizar en un primer tiempo operatorio la curvatura y .el espesor de la córnea mediante una quera~ to plastia total superficial.
El curso post-operatorio de estas in· tervenciones fué muy simple y contrariamente a lo que esperaba, la córnea permaneció completamente transparente permitiendo U'1a visi6n excelente con una corrección, en unos casos hiperme. trópica (+ 2) en otras mi6pica (- 3).
En operaciones sucesivas y en la operaci6n del segund o ojo de estos paci en~
tes, me fué posible acercarme mucho a la emetropía, vigilando a l oftalmómetro el radio de curvatura corneal y modificándolo con el uso más o menos prolongado de un vendaje compresivo

com.o si de una ectasia del injerto se tratara.
A fin de simplificar aún más el curso
post~operatorio recurrí a la autoplastia en casos de pequeños 'querafocorios en los que la ausencia de a lteraciones de transparencia, permitían la práctica de una intervención modificadora de la curvatura corneal. sin necesidad de recurrir o un material homoplástico que
aunque bien tolerado, lo es siempr,e menos que el aufoplástico.
La técnica seguida fué simple, con un
trépano de I I mm,. se delimit6 un lecho de la mitad de la profundidad del espesor corneal. A continuaci6n con un trépano de 10 mm. se delimit6 un nue· vo lecho concéntrico al primero. Con un fino cuchillete se extirp6 el anillo corneal de '12 mm. delimitado por las dos trepanaciones. procediéndose a continuaci6n a disecar foda la porción central com9 si de una .querafoplas+ia total superficial se tratara, pero suturan· do nuevamente en su lugar el injerto asf ,obtenido, previa para'centesis, con pun· tos córneo-cornea les situados en el limbo, y en número de 12 a 16 según los ,casos.
El curso post-operatorio fué aún más r6pido que en los operados de homoplCistia no presentándose en 'ningún case el edema hipotr6fico, que es casi constante alrededor del 20.0 dra en las ~omoplastias totales superficiales, lo cual no es de extrañar por tratarse de 'material auto plástico.
La membrana de Descemet y capas
profundas de la c6rnea, se adaptan perfectamente a su nuevo radio de curvatura sin que en ningún caso haya podido observar la formaci6n de pliegues.
En vista de la inocuidad de la intervenci6n, la practiqué a continuaci6n en c6rneas normales de suj.etos afectos de anisometr.opía de unas 10 dioptrías obteniendo excelentes resultados, por lo que considero que el método puede aplicars,e con éxito a la correcci6n de las ametropías puras, evitándose de esta forma ,el uso de cristales de contacto y de cristales correctores de alta graduaci6n, pudiendo también solucionarse una cuestión hasta la fecha insoluble como la de los afáquicos monoculares y la de las anisometropías por miopía monocular, además de la cuestión estética del uso de cristales corre~tores de alta graduación ya antes reseñada.
CORRECCION DE AMETROPIAS
La mayor parte del poder r,efringente del sistema 6ptico ocular corresponde a la córnea siendo éste, función de su curvatura .
Según Tscherning, de las 58' 17 diop. trías que representan el total de la refringencia ocular, 42'51 corresponden a la cómea. Modificando pues la curvatura de esta membrana, dispondrem"os de un amplio margen, más que suficiente para corregir cualquiera de los grados de ametropía que habitualmente se presentan en clínica.

disminuyéndose considerablemente sus
riesgos, pues las capas profundas se adaptarán fácilmente a la forma de las
superficiales. El proceder quirúrgico es dif,erenle
según debamos disminuir o aumentar el poder refringente de la c6rnea,

bre todo un interés teórico. Por lo que
considero impropia su exposición en lo
presente nota previa.
Actua lmente tengo en estudio la confección de unas tablas para sim
plificar la soluci6n de cada caso.
Suponiendo por ejemplo que el anillo a extirpar es de 112 mm. de anchura,
procederemos de la siguiente forma:
Con un trépano de 11 mm. delimitaremos un lecho como si de una quera
toplastia total no penetrante se tratara,
profundizando 112 o % del espesor corneal: misma maniobra con un trépan.o
de 10 mm. procurando qued,en embos
delimitaciones bien centradas (Fig. 2-A). Con la punta de un lanceolar o cuchi

cos, procurando hacerlo a una igual
profundidad en Ioodo el perrmetro (Fig. 2-B). Mediante el escarificador de 1<uhnt, disecamos lo porci6n central
monteniendo siempre el mismo plano
(Fig. 2-C). Sin cambiar su posici6n con rli'Iaci6n al meridiano horario y previa paracentesis de la cámara anterior se
procede a suturarlo al limbo por medio
de puntos sueltos c6rneo-corneales en
número de 12 ó 16 según' los casos, re· partidos uniformemente por su períme
tro. (Fig. 2-0). En 105 pequeñas corr.ecciones no pre
cisa despegar la totalidad de la córnea, puede respetarse la porci6n 6ptica central. Este proceder parece a primera
vista tener ventajas sobr,e la disección
total, pero esta última no me ha deter ..
minado opacidades apreciables y tiene
la ventaja de no precisar el uso de trépanos concéntricos hoy día de difícil
adquisici6n.

rio, con lo que se facilitará grandemente la práctica de op.eraciones para pe~ queñas correcciones a la par que au· mentará la exactitud de éstas.
La paracentesis de la cámara anterior es convenient.e para evitar la tracción excesiva sobre los puntos d.e sutura. En las grandes correcciones puede ser conveniente la práctica de un Elliof tempo"ral.
El curiO post-operatorio es simple, no precisáridose el uso, en g,eneral, de los midriáticos. Los puntos se retiran al décimo día: en los sucesiv,os se comprueba al oftalmómefro r el grado de curvatura corneal que s.i aún es excesivo, po-
dró disminuirse con la aplicación de un vendaj,e compresivo. dejando el otro ojo
descubierto. Para simplificar esta técnica oon la
cual he intervenido los primeros casos, tengo actualmente en experimentación un nuevo dispositivo. El modo de proceder con el mismo es el siguiente:
E.n el ojo a intervenir, con un trépano de II mm. se delimita un lecho con una profundidad de 112 a % del parénquima corneal, procediéndose seguidamente a extirpar las capas anteriores de la c6rnea como si de una queraloplastia superficial total se tratara.

a un molde convexo y de la curvatura adecuada.
Por medio de una fijación especia l, la eórnea queda sujeta y con un trépano también de 1I mm. se extirpa la lona periférica sobrante.
Este método tiene una gran ventaja , 10 de no necesitar una gran variedad de trépanos concéntricos y simplificar la intervenciÓn, ya que el momento más delicado de la misma, o sea la extirpa ~
ci6" del anillo corneal periférico, puede hacerse fu,era del ojo receptor y el instrumental debe consistir solamente en una serie de moldes de las diferentes curvaturas cornea les, instrumentol que por otra parte, tiene la ventaja de po· der ser utilizado infinidad de veces sin necesidad de reparaciones.
Esta técnica tiene también la ventaja de permitir prescindir de los cálculos matemáticos ya que los datos propor· cionados por el oftalm6metro son SUM
ficientes.
CORRECCION DE HIPERMETROPIAS
Para aumentar el radio de curvatu~
ro en las pequeñas ametropfas, técni~
comente podría procederse en la misma forma que para las miopías pero a la inversa o sea adicionando un pequ,e~
ño a nillo de c6rnea, en este caso homo M
plástica en todo el perímetro corneal. Esta intervenci6n parece complicada
y por ello no la he experimentado aún: sin embargo, técnicamente parece ser la mejor.
Con queratoplastias parciales, para

concretar de 6 mm., es posible obtew
ner correcciones bastant.e elevadas (hasta 15 dioptrfas) disecando la c6r.
nea del receptor hasta muy cerca de la Descemet (fig. 3.A.B) Y substituyéndola por un injerto de espesor uniforme, unas décimas de milímetro mayor que el lecho (Fig. 3.C). El cálculo es el mismo que en el caso anterior pero substitu M
yendo el valor de (m) por 3 en lugar de considerarlo 5'5 (m es la mitad del diá· metro del trépano).
Este método es el que en la práctica

me ha dado mejor resultado. Emplear injertos homoplásticos de mayor dimE..n~
si6n no parece prudente por la posibilidad de presentarse ligeros 'opacidades hipotr6ficas post-operatorias, al mismo tiempo que requerirían una mayor distensi6n de la membrana de Descemet y
capas corneales profundas amén que las correcciones de más de 10 dioptrías 5610 pueden obtenerse, dado ,el espesor corneal, oon un diámetro reducido.
.&:Actualmente tengo en experimenta
ci6n dos métodos más que posiblemen-
te permitirán una moyor aproximaci6n en la corr.ecci6n di6p+rica.
El primero consiste en tallar un Je
cho de forma oonvexa m.ediante una
fresa especial y exact~mente de la curvatura necesaria para cada caso (Figu
ra 4). Un injerto laminar de espesor· unifor
me lo recubre (Fig. 4-C). El segundo consiste en tallar un le
cho normal como en una queratoplastia
laminar oorriente (fig. S-A-B) Y colocar
entre él y el injerto una lente de mate
ria pl6stica (fig. S-C) para que manten
ga la curvatura deseada de la cara an
terior de la c6rnea (fig. 5-0) Y aprovechar al mismo tiempo su fndice de
refringencia que serra conveniente fue
se elevado.
Las c6rneas asf obtenidas se aseme
jan a los cristales de contacto de Zeiss para hipermétropes o sea que presentan una pequeña saliencia de 6 milímetros de diámetro en la cual el radio de curvatura es 'm,enor que en el resto de la c6rnea.
RESUMEN
La experiencia del autor en queratoplestia. le ha sugerido la posibilidad de corregir los defectos de refracci6n por me~:Ho de interv,encion es plásticas s'obre la c6rnea.
Ha realizado con éxito dicha intervenci6n en casos de anisometropía, habiendo empleado técnicas de autoplastia no penetrantes cuyo fundamento se

expone en la presente nota previa y con los cuales el riesgo quirúrgico es mínimo.
La autoplastia en las miopías, con resultados comprobados y la homoplastia en la correcci6n de afaquias.
Se exponen también métodos actualmente en experimentaci6n con los que posiblemente podrá conseguirse más fácilmente una emetropía perfecta.
KÉRATOPLASTIE RÉFRACTIVE

Ce dernier probleme étont oujourd'hui résolu, nous ovons commencé el oppliq,uer lo kérotoplastie el des yeux soins et meme nous pou
. vons la pro po ser comme I'intervention la plus oppropriée peur modifier la réfroctien des yeux ométropes n'oyont aucune lésion de la carnée. Aux quot re fins peursuivies en pratiquant une
kéroteplastie, nous pouvons ajouter une cin-quieme, la f in réfroctive. •
Ainsi les kérotoplos+fes ont une fin: Optiq,ue. Reconstructive. Théropeutique. Es+hétique . Réfracfive. En pratiquant des kératoplosties sur des cor
nées opaques el fin optique, nous ovons pu observer des degres tres élevés de myopie dons des greffes qui étaient restées legerement bombées et des hypermétropies, quand en raison de la cicatrisotion, la cornée restoit oplatie. Aussi en 1948, nous conselllómes de pratiquer des kérotoplosties optiques sur des yeux myopes en employont une greffe un peu plus petite que la tréponotion el f in d'augmenter le rayan de courbure de lo cornée et, en réduisant son pouvoir dioptrique. corriger ou tout au moins dimi nuer I'amétrop ie.
De cefte maniere nous avons pratiqué de nombreuses opérations avec succes.
De meme, chez des malades qui présentaient des opacités de crista llin et qui apres la kératoplastie devoient étre opérés de la cataracte, nous avons placé une greffe un peu plus grande a fin d'augmenter la courbure de lo
cornée et corriger en portie I'hypermétropie quí résulta ít de I'aphakie.
Ces malades furent opérés suívant lo technique pénétrante étant donné le risque, quí, bien que petH, existe toujours, nous n'avons pos cru prudent de procéder el une kératoplastie sur un m il a cornée saine dans le seu l but de modifier son amétropie.
L'examen des molodes atteinh de kératoconus opérés de kératoplastíe pénétrante de /) mm. nous poussa O: protiquer des kératoplastíes de plus grande importance O: f in de régulariser completement la courbure de la cornée, car, mame chez ceux sur q~i nous obtenions d'excellenh résultah visuels, il subsistait souvent une légene amétropie, due O: ce que la greffe restait posée sur la pointe du cóne. Castro vie jo, dans une de ses récentes étude"S, insiste particul ierement sur ce faH et cite des cos quí ont nécessité trois interventions de grandes d imensions pOur obtenir !Jne régularisation de la courbure.
O'autre port, !,étude de I'hypotroph ie de la greffe nous indiqua qu' il est préféroble de ne pos pratiquer des kératoplasties pénétrantes de plus de /) 112 mm. et la nécessité de foire la greffe aVeC un tinu cornéa l d'épaisseur normole. Ces conditions ne pouvant atre réun ies dans les cas de grands kératoconus, nous décidámes de régulariser, dans une premiere phase opératoire, la courbure et I'épaisseur, ou moyen d'une kératoplastie totale supe nicielle.
L'évolution posf-opératoire de ces interventions fut tres simple et controirement a notre attente, la cornée resto completement transparente, permettant une exce llente vision ovec une correctíon hypermétropique (+ 2) dans quelques cas et myopiques (- 3) dans d 'autres.
Oans des opérations succesives et dons J'opération du second ceil de ces malades, il nous a été possible d'atteindre presque I'ométropíe en surveillant O: I'ophtolmometre le rayan de courbure de la cornée et en le modifiant au moyen d'un bandage compressif appliqué plus ou moins longtemps, comme s' il 5'0-

Suivant Tscherning, sur les 58'17 dioptríes représentont la réfringence totole oculoire, 42'51 correspondent a lo cornée, En modificiont donc la courbure de cette membrane, nous disposerons d'une grande marge plus que suffisonte pour corriger tous les degrés d'ométropies qui se présentent habituellement en clinique .

Le ptocédé chirurgical est diffétent selon q,ue I'on doit diminuar ou augmenter le pouvoir réfringent de la cornée.

CORRECTION DES MYOPIES
Pour augmenter le royan de courbure de la cornée, il est nécessaire d'extirper sur la pétiph érie de la cornée, un onneau oyont la lorgeur correspondant au degré de correctio n que I'on désire obtenir et dons la profondeut de Ih a 14 de I' épo isseur de la cornée.

CORRECTION DES HYPERMETROPIES
Pour ougmenter le royon do courbure dons les petites ométropies, on pourroit procédet de la meme maniere que dons les myopies, mais en sens in verse, 'S 'est a-d ire, en a ioutant un petit onneau de cornée, dans ce cas homopladique, sur tout le périmetre cornéa l.
Cette inte rvention parait compliquée, oussi ne I'ovons nous pos encore expérimentéej cependont elle semble etre techniquement la meilleure.
Avec des kératoplosHes partielles, par exernpie de b mm., ji est po'Ssible d'obtenir des cortecHons auel élavées Ijusqu'a 15 dioptrias) en disséquant presque la cornée du técepteur jusqu'o celle de Descemet (figs. 3-A-S) et en la remploo;ont par une greffe d'épaisseut égole de quelques dixiemes de mm. de diometre plus gra nd e que I'onise (fig. 3-C). le co lcul est le meme que dons le cas ontérieur mois en substituont la va leur de (m) par 3 au lieu de lui donner 5'5 (m est lo moitié du diometre du trépon).

De'S méthodes actuellement el I'éssai avec le squelles on pourra peut-etre obtenir plus facHement une émétropie parfaite, y sont exposées.
REFRACTIVE I(ERATOPLASTlC
(PRELlMINARY lNFORMATlON)
In the beginnings of keratoplastic operations, the attention of the surgeon concentroted mainly on obtaining the adherence of the graft whereas its transparence, more or le 55, was given -secondary consideration anly. With the perfection af surgical techniq,ue, the adherence of th e graft ceased to be a problem and in its stead the task of obtaining satisfactory transparence had to be solved. As the lotter question 0150 ha s found its solution today, th e application of keratoplastics to eyes in otherwi'Se perfect condition, has been begun and to such an extent, that 1 now may propase keratap lastics as an intervention to be performed in orden to change the refraction af ametropic eyes, which da not show any defect of the Corne"a.
T o the four previous cases, in which keratoplastic seems indicated, we now can add faulty refradion as a fifth case.
We may now soy that we recognize keratoplastic operations as being practised to the following e nds:
Optical. Reconstructive . Therapeutical . Aesthetical. Refractive. On the occasion of the . performance of ke
ratop lastic operations on dim corneo, to aptic ends, we had occasion to observe myopia in grafts which had become slightly inflated and, on the other sida, hypermetropia in· those cases in whic h the corneo had been flattened by a cicatrization process.
This caused me to advise, in 1948, to perform optical keratoplastics of myopic eyes in such o manner, that a graft would be inserted, which were a li+tle thinner than the trepanation, with the aim af increasing the radius af the cornea l curve and, thereby, of removing the ame tropia or, at least, of diminishing its degree partly.
Proceeding in this manner I interve ned in severa l cases, which gave satisfactory results. I 0150 inse rted grafts into' the eyes of several patients, who showed dimness of the cristoline and who had to undergo operations because af existing catarocts, after a keratoplastic operation had been performed on them. These grafts were s lightly larger, in order ta increase the corneal curve and in arder to ame liorate partly the hypermetropia caused by their aphaky. The operations on these patients were performed using penetrotion-techniqua, as, considering the only slight, but nevertheless existing risk, I did not deem it justified to regard the indication for o kerotoplastic of an eye with normal cornea as given, the only intantion being to axert an influe nce on the existing ametropia.
The examination of kera toco nvs-patients, who had undergone e penetrating kerataplastic operation of 6 mm. and who, despite exellent visional results, someti mes had been left whith a slight ametropia, partly due to the clinging of the graft t o the point of the co nus, gave me the impression that it should be advisable to perform kerotoplastic operations of larger dimensions, aiming at the complete normalization of the' cornea l curve.
Castrovieio, in one of his recent publicaHons, insish especially on this detail and cites cases, which needed three maior operotions before a normalization of th e cornea had been achieved.
On the other hand, examinotions of hypertrophy of grofts showed me the advisability of not performing penetroting kerotoplostic opetotions of more thon b 112 mm. and I olso found that it wos necessary to delimitate the grafts with corneal tissue of normal thickness. These conditions cannot be reunited in coses which show o big ker,atoconus, becou"Se of which I decided to correct the curve and the thickness of the corneo in o first operational phose by complete. superficial kerotoplostic operotion.
The post-operative course of these interventions was very simple, and, contrary to my expectations, the cornea, temained completely tronsparent and permitted exellent vision with hypermetropicol correction of 1+ 2) ond with a cOfredion of myopio of (- 3 J in other coses.
During the following operations and during the operation on the other eye of the patient, I succeeded to a great extent in achieving emetropia, by "Supervising the corneal curve with the aid of an ophtholmometer and by influencing this curve through the longer or shorter USe of a compression-bandoge, as if it wete an ectasy of the gran.
In order to simplify the post_operotional course eVen more, I used autoplostic operation in 011 cases in which the lock of modificotions of transporence permitted the execution of an operation modifying the cornea. Thus, it was not necessary to use a homoplastic material, which, even if it is tolerated well, 1s alwoys less 1ndicated thon autoplastic material.
The pt.ocedure followed was rather uncom plicated: A bearing, which was helf as thick as the t"¡ckness of the corneo, was cut with the oid of en eleven-millemeter trepan. Then a second bearing wes cut concentricolly to the first, this time with the use of a ten-millimeter trepan. Then o 112 mm. ring of the cornea, delimitoted by the two preceding trepanations, IWOS cut with the oid of a very thin lancet, 011 this being followed by the complete extirpation of the entire, central portian, as if we were treoting a complete superficial keratoplastic, only that in this case the groft, which had been thus obtoined, wos once more sewn to its place. The corneo-corneol sutures, their number varying between 12 and lb, according to the case, were located in the margin and the operation had been preceded by a parocente"Sis.
The post-operative course was 011 the more ropid, os none of the p'atients, who had been operoted on occording to the homoplastic
method, showed cny signs of hypotrophye edema, the occurrence of which on the 20th doy following the operation may be considered aS a rule, which is not surprising, as the material used ís autoplastic.
The membrane of Descemet ond the deeper layers of the cornea odapt themselves completely to the new radius af the corneal curve, without OUt having been able to observe the occurrence of wrinkles in any case.
The intervention being undangerous I used it currently on the normal cornea of per~ sons who otherwise showed anisometropia of obout 10 dioptries.
As the results Wete exellent I believe that this method may be used suceuful1y in the correction of pure ametropíes, thus avoiding the use of contactlenses or of high-degree glasse'S. It would 0150 help to salve the until today unonswered problem of monocular aphaky, of monocular myopic anisometropies.
CORRECTION OF AMETROPIA
The greater part of the refractive powef of the optical systems is coused by the corne'al curVe referring to this quality.
According to Tscherning, we owe 42'51 dioptries to the corneo, whereo5 the total of re~ fractive power of the eye consists of 58'17 dioptries. The modification of the curve of this membrane gives us an ample margin, which is more than sufficient to corred any degree of ametropía in the material usually found in clinics. If we considet the cornea to be the segment of a curve, which at its ends is connected as if by a string, the lengthening or shortening of this segment will inevitably modify its curve. If we shaden its extent, the curve wilt be narrow, affecting the diophi cal power, and, if we lengthen it, refradion will be modified in the inverse san se. The dioptrical power of the cornea is about entirely concentrated in its frontal surface (47'42 dioptries) and ,only (-4'73) ore caused by the dorsal surface. By dividíng tlle cornea through the middle of its parenchym into two halves, We "Shall see, that the elostic qualities of each holf are quite different. Due to a greoter concentra tia n of corneal laminae Clnd due to the membrClne of Bowman, the frontClI half is only li+tle elastic. The dorsal half, formed by the deep layers of the corneal elements and by Descemet's membrane, is much more elostic,

due to the qual ities of this membra ne ond due to the loxn es5 of the deep cells of the corneol parenchym. This fad has repeatedly hean observed by 011 those acostumed to perform nonpenetroting keratoplo'Stic operotions.
These qua liti es of the different layers of th e cornea permit us to modify its curve withouf beeing obligad to approoch the entire cornea surgically. Th i5 pe rmits on edra-ocular operation, thus diminishing its risk considerably. The deep loyers of the corneo occomodate themse lves eosily to the surraca layers.
The opera tional procedure d iffers, occording to the question ba lng aithar t he ¡ncrease of t he refractiva power of the cornea or its decrease. In the following , t 5hall give detailed infermotion concerning t he methods tried by me and which oim ot the corredion of 0$ well myopia os hypermetrapia. Asti gmatisms may be treated using the, same methad, but they ·will be my concern on sorne other occasion.
CORRECTlON OF MYOPIA
In arder to augment the radius of the corneal curve, it is necessary to cut out a ring in t he periphery of t he cornea, the width of which is determi ned by the degree of correction to be achieved and the depth of wh ich be 1/2 - 3,4 of the thickness of the cornea. The width of the ring mentioned, wh ich has to be calculated in every single case, is determined according to the following formula:

I am adua lly preparing sorne tables, which will simpl ify t he coteu lations to be done in eoch single case. Assuming thot the ring to be cut out is 112 mm. wide, we proceed '05 fo llows:

Without any change of its position, as far as the hour-m'i!ridian is concerned , and only 'after a poro' centesis of th e anterior chomber has been performed before, morginol fi xation is performed with the oid af abaut 12-16 loase corneo-cernea l sutures, distributed regularly throug hout the perimeter (fig.2.o).
Sma ller carrections do not afford a loosening of the cornea in its entire extent and thc optical centra l portio n m'ay be left in its pesitio n there in. The procedure see ms to offer certoin advantage'S at first right, when compared to a complete losening, but I hove never been able to observe oppreciable dimness in the latter, and, besides, it has the advantage that conce ntric trepans are not used in it, os these ore difficult to obtain nowadoys.
In order to 'Si mplHy this ehapter ef the aperation, 1 have had a specio! knife constructed, whieh immedietely cuts a ledge of the neces· sary width, a fad which will not o nly fociBtate the ope rotion, but 0150 increase its precision.

importont corrections, o temporal operation according to Elliot might be indicoted.
The post-operative course is simple and uncomplicated, os, in generol, the application of midriatica is not required. The sutures ore removed on the tenth doy following the operation. On the following days, the corneol curve is meosured with the oid of an aptholmometer. If this curve is exoggerated, it can be reduced by the use of a pressurebandage not covering the other eye.
In arder to simplífy the technique which 1 used in the performance of the first operations of this kind, 1 am adually trying out a new procedure os follows: '
In the eye to be treated, a bearing of 1/2-% the thickness of the cornea is cut, using on 11 mm. trepan, and, as in o complete superficial keratoplastic operation, the anterior la yen of the cornea are extirpated subsequently. The thus obtained groft is put into a convelC form, which has been adapted to the corneal curve of the eye concerned.
The cornea is held in ib place by a special method and now the above cited peripheral lone is elltirpated with the aid of also an 11 mm. trepan.
This procedure has the odvontage of not requiring a great number of different concentrical trepans and of thereby simplifying the intervention, as the elltirpation of the peripheral ring of the cornea, which is ib most delicate phase, may be performed oubide of the receiving eye. The inventory of instruments need only consist hetein of O' se ries of forms adap1ed to the different corneal curves and these forms have the further advantage, that they may be used an infinite number of times without needing any kind of repairs.
This technique offers the further odvantage of rendering superfluous the mothematical calculations, the use of the proportional figures of the ophthalmometer bejng sufficient.
CORRECTION OF HYPERMETROPIA
In order to augment the radius of the corneal curve in cases showing only slight ametropia, we may technically proceed as follows in the chapter concerning myopio, only in the inverse sense, which ¡s, by adding a narrow cor neal ríng by homoplastic operotion in the whole elltent of the corneal perimeter.
This intervention appears to be very difficult and 1 hove therefore not described if yet, even if it seems to be the best woy of aperating, from a technical point af view.
The performance of an incomplete keratoplastie operation, of specifically 6 mm. makes jt possible to achieve high deg rees of correctia n lup ta fifteen dioptríes). by ellfirpating the cornea of the reeeiver until close to the membrane of Descemet (figs. l-A-B) and by replacing it by a graft of equal thickness, which should ho ve a diametet of 'Severa l tenths of millimeters more than the bearing. IFigs. 3-CJ. The calculations ore the same as in the cose eited befare. only the value of 1m} shoutd be 3, instead of 5'5 (m being half the diameter of the trepan).
This method has shown the best practical results. As the occurrence of slight, hypertrophjcal and post-operative dimness may occur, it seems to be advisable, not to use homoplosfic grafts exceding these dimensions. They would also requifa a greater elCpansion of the membrane of Descemet and of the deap layers of the cornea, whereos more important corrections of more thon 10 dioptríes, con, in view of the thickness of the cornea, be only achieved with reduced diameter.

Th e first method eonsish of cutting o convex bearing with the aid of a special insfrument elCactly adaptad to the curve required in each case. IFig. 4).
This bearing is coveted by a disk-shaped groft of uniform thickness. (Fig. 4-C).
The second method eonsists of cutting a normal bearing, as in an ordinary laminary kerotoplastic operation (Figs. S-A-B), and by now insetting between this boaring and the groft a lensa mode of plastic material, in arder to maintain the curve desired in the frontal surfoee of the cornea. Besides we use its refradion index, which, in arder ta be of adva ntage should be high.
The cornea es, which hove thus been obtoined, ressemble the Zeiss-contaet-Ienses for hypermetropia, that ¡s, they show a little protubetanee of b mm. in diameters, the curval radius of which is smaller than that of the red of the corneo.

REFRACTlVE KERATOPLASTlK
(VORLAEUFIGE MITTEILUNG)
In den Anfoengen der Keratoplastik richtete sieh die Aufmerksomkeit des Chirurgen in enter Uníe darauf líe Adheren des Transplantates IU erreichen, wobei die mehr oder weniger grasse Transparenz des Tronsplantotes erst in ~we¡ter Unie Beochtung fondo Mit der Ver. vollkommnung der chirufgi'Schen Teehniken war jedoch die Adheren~ des Transplantates kein Problem mehr und an seine Stelle trat die Aufgabe, eine :zufriedenstellende Transparent J:U 'erreiehen. Nachdem heute diese letrlere Frage geloest ist, ist damit begonnen worden, die Keratoplastik aueh bei Augen in beuerem Zustand ontuwenden und twar so weitgehend, doss ich jent die Kerotopla'5tik 015 Eingriff :zur Aenderung der RefroUion van ometropen Augen, die keine laesion der Cornea aufweisen, vorsehlagen konn.
Zu de n vier Foellen, in denen eine Kerotoplastik als angebraeht erscheint, koennen wir ols fuenften Fo il d ie refraktaere Indikation hinzufuegen.
Wir kennen info lgedenen Kerotoplastikon, die mit der folgenden Zielsenung vorgenommen werden :

trueben Corneen, mit optiseher Zielsenung, haben wir hoehgradige Myopie bej Transplan"taten, die etwas 'Oufgeblasen waren, beobachten koennen und dagegen Hypermetropie in Faellen in denen dureh den Varnarbungsprotess die Cornea abgeflaeht war. Infolgedessen riet ieh 1948, die optisehen Keratoplastiken be; myopen Augen so au5tufuehren, don ein Transplantat einge'Sent wurde, welches etwas duenner wor ols die Trepanation, mit
dem Ziel, den Kruemmungsradius IU ve rgroesS8rn und dadureh die Ametropie ~u beseitige n oder wenigdens teilweise ~u verringern.
Auf diese We;S8 nohm ieh in verschie denen Faellen Eingriffe mit :zufriedensfellenden Resultaten vor.
Ebenfalls sente ieh beí einigen Patienten , die Truebungen des Kristallkoerpers :zeigten und an denen noeh der Keratoplastik EingriHe wegen bestehender Katorakte vorgenommen we rden muuten, Transplantate ein, die etwos groesser woren, mit dem Zie l, die Kruemmung der Cornea IU vergroessern und die Hypermefropie, die eine Folge der Aphakie war, teilweise :zu verbenern. Die Eingriffe an diesen Patienten wurden mittels Penetrationsteehnik vorgenommen, we il, in Anbetraeht des twar geringen, aber immerhin vorhandenen Risikos, ieh es nieht fuer riehtig hielt, die Indikatian :zur Keratoplastik be; einem Auge mit gesunder Cornea als gegeben :zu befraehten, :zu dem ein:zigen Zweek, d ie Ametrop ie :zu beeinflussen.
Die Untersuehung der Keratokonus.Patienten , an denen eine penetrierende Keratop ladik von b mm. vorgenommen wurde und bei denen, tron ausge:zeichneter visueller Resultate, :zu ~ weilen aine leiehte Ametropie :zurueckbleib+, welehe teilweise darauf :zurueehufuehren ist, dan das Tra nsplantat sieh auf der Spine des Konus festset:zt, lie'Ss es mir ange:zeigt ersehé inen, Keratoplastiken groesseren Ausmass vorJ:uehmen, mit dem Ziel, die Kruemmung der Corneo vollkommen :zu normal isieren.
Castravieio besfeht in einer seiner lente n Veroeffentliehungen besonders ouf dieser Ein~ :zelheit und fuehrt Faelle an, in denen drei groessere Eingriffe noetig waren, bis e ine Normalisierung der Cornea a rreieht war.
Andererseits lieuen mir U ntersuehungen von Transplantaten auf Hypertrophie angezeigt erseheinen, keine Penetrations-Keratoplasfiken von mehr a ls 6 112 mm. aus:zufuehren und ich erkannte, dan es notwendig ist, das Transplan~ tot mit Corneagewebe von normaler Oicke IU begrenten. Diese Bedingungen koennen in Fael1en von grossem Keratokonus nieht verein· igt werden, we'Swegen ieh mieh da:zu entsehloss, in ainer arden Operationsphase die Kruemmung und die Oide der Cornea dureh eíne totale Oberflaeehen-Keratoplasfik tU be· riehtigen.
Der post-operative Verla uf dieser EingriHe war sehr einfaeh und, im Gegensatz :zu meinen Erwortungen, blieb die Cornea vollkommen transparent ul1d erlaubte ein ausge:zeichnetes Sehtermoegen mit einer hypermetropen Betiehtigug von 1+ 2) und einer myopen Berieh-
tigung van (- J) in andoren Faellen. In noehfalgenJen Operationen und be i der Operation de'S anJeren Auges des Potienten ge long es mir weitgehllnd, die Emehapie IU erreichen, wobei ieh mit Hilfe des Ophtalmometers den Kruemmungsradius der Cornea ueberwaehte und diesen dureh den mehr oder weniger langen Gebraueh eines Druekverbandes veraenolerte so, als ob es sieh um eine Ektosie des Transplantates hand e lte.
Um den post-operativen Verlauf noch mehr IU vereinfaehen, griff ich in Faellen van kleinem Keratokonus, bei denen das Fehlen van Veraenderungen der Tra nsparenz die Vornahme eines die Kruemmung der Co rnea madifilierenden Eingriffs erlaubte, auf die Autaplastik zurued, so das$ es nicht notwendig war, ein homoplastisches Material zu gebrauchen, welches, wenn es auch gut vertrage n wird, immer wcn iger gut ist als das autoplastische.
Die angewendete T eehnik war denkbar einfaeh : mit e inem Trepan van 1I mm .. wurde ei n lager ausgeehnitten, welehes halb so tief war, wie die Díde der Cornea. Dann wurde mit einem Trepan van 10 mm. ein zwe ites loge r Konzentrisch zum enten ongelegt. Sodann wurde mit einem feinen Meuerehen ei n 112 mm. breiter Ríng der Cornea, begrenzt dureh die beiden vorangegangenen Trepanationen, angelegt und in der Forhehung die gesamte Mittelpartie ausgeschnitten, als ,ob es sieh um eine totale Oberflaechen-Keratoplastik ha nde lte, wobei jedoch das so erhaltene Transplantat neuerlich an seinem Plan festgenaeht wurde, wobei sich die corneo-corn ea len Naehte, je nach Art des Falles 12-16, am Ronde befanden. Eine Parazentese war vorausgegan,gen.
Der p'ost-operative Verla uf war umso schne ller, da bei de n homeoplastisch operierten Kranken - in keinem Falle ein hypedrophes Oedem auftrat, we lches bei der t ota len Oberflaechen-Homeoplastik am 20. Tage sozusagen di e Regel ist, wos nieht verwunde rn darf da es sich jo um outopladisches Moterial hond elt.
Die Descemt'sche Membran und die tiefen Schiehten der Corneo pouen sieh vollkomme~ on den neuen Kru emmungHad iu'S an, ohne das, wi-r in irgendeinem Falle das Auftreten van Fa lten haetten beobaehten koennen. '
In Anbetraeht der Ungefaehrliehlteit des Eingriffs fuehrte ¡eh ihn fortlaufend on normalen Corneen von Personen aus, d ie eine Anisometr~pi~. van e inigen 10 Dioptrien aufwiesen • . , Die Ergebnisse waren ausgeze iehnet, weswegen ieh glaube, dan sieh die Methode mit Erfolg lur Verbesserung yon reinen Ame-
tropien andwenden laout, wodureh der Gebroueh van Kontakt- Unsen ader von hochgradigen Glasern verrnieden wird. Auch koennte so das bis heute ungeloeste Problem der monokularen Aphokien und der monokularen myopen Anisomehopien geloest werden, ganz abgesehen von der Frage des kosmetischen Aspektes des Gebrouehs van hochgradigen G lasern, wie schon vorher besproehen.
DIE KORREKTlON DER AMETROPIEN
Der groesste Teil des Breehungsvermoegens des optischen Systems beruht auf der diesbezueglichen Eigenschaft der Kruemmung der Cornea.
Naeh Tseherning werden von den 58' 17 Dioptríen, w~ le h e zusammen das gesom te Brechungsvermoegen des Auges darstellen. 42'5 1 Dioptrien von der Cornea aufgebraeht. In der Veraenderung del Kruemmung dieser Membran besinen wir e in en Spielraum, der mehr als ausreiehend ist um jeden Grad von Ametropie n zu korrigieren, die sieh im Allgemeinen in den Kliniken vorfinde n.
Wen n wir die Cornea 015 das Segment eines Bogens betrachten, we lches an seinen Enden wie mit einer Schnur verbunden ist, so wird die Verlaengerung oder Verkuenung des Segmenfs gezwungenermanen seine Kruemmung ve raende rn. Wenn wir die Laenge verkuerzen, wird sieh die Kruammung verkleinern und damit di! Diophienstaerke, we nn wir sie dagagen verlaengern, wtrd sieh die Brechung im umge¡ keh rten Sinns veraende rn. Die Dioptrienstaerke der Corne a beruht faS+ ganz auf ihrer Vorderflaeche (47'42 Dioptrien) und nUf (-4'n ) werden von der Hinterflaeche a ufgebraeht. Wenn wir die , Corn ea dureh die Mitte des Parenehyms in Haelften te ilen, w.erden wi,r sehen, dan die e lastisehen Eigensehaften jede-r Haelfte sehr verse hieden sind. Auf Grund einer groe'SSeren Ansammlnug van eornea len l amellen und der Bowman'schen Membran, ¡st die vordere Haelfte se hr wenig dehnbar. Die hintere Haelfte, die von den tiefen Schie htan der cornea len Elemente und von der Descemet'sehen Membran gebildet wird , ist auf Grund der Eigensehaften dieser Membran und ouf Grund der groesseren Schlaffheit der tiefen Zellen des cornea len Parenehyms wesentli ch elastiseher. Diese Tohache ist mehrfoeh von Allen beobaehtet worden, die Erfahrung in der Ausfuehrung von nicht-penetrierenden Keratoplastiken haben.

Das operativa Vorgehen 1st verschieden, je nochdem oh es sie h darum hondelt die Brechungskraft der Cornea zu vergroessern oder zu verringern. Nochfolgend werde ¡eh 1m einzalnan d ie Methoden mitteilen die ¡eh erprobt haba und die zum liel haben, die Myopien und die Hypermetropien zu korrigieren. Astigmatismen koennen nach detselhen Methode behandelt werden, ¡eh werde mich jedoch mit denselben bei ande rer Ge legenheit beschaeftigen.
KORREKTlON DER MYOPIEN
Um den Kruemmungsradius der Cornea IU vergroessern, ist es notwendig in der Peripherié -der Cornea einen Ring anluschneiden, dessen Breite sich nach dem angestrebten Korrektions-Grad richtet und de'Ssen Tiefe Ih _ % der Dide der Cornea betraegt. Die Breite des besagten Ringes, die in jedem Falle berechnet werden muss, wird nach fo·lgender Formel berechnet:

Kruemmung der dorsalen Flaeche der Cornea, jedoch sind diese Veraenderungen geringfueging und haben lenten Endes nur theoretisches Inte resse. Ich halte infolgedessen ihre Dardel1un g im Rahmen dieser vorlaeufigen Mitteilung fuer nicht angebracht.
Augenblicklich bearbeite ich einige Tabellen, welche die Berrechnung in jedem Falle verelnfachen. Wenn wir annehmen, dass der aus'Iuschneidene Ring 112 mm. breit ist, so gehen wir folgendermassen vor :
Mit einem 11 m. Trepan bereiten wir ein Lager wie bei der totalen, nicht-penetrierenden Keratopla'Stik, van einer Tiefe von 112 - % der oide der Cornea. Dann folgt dieselbe Handlung mit einem JO mm. Trepan, wobe i beide Ausschnitte gut konzenhisch lie gen muessen (Fig. 2-A ). Mit der Spine e iner Lanzette oder eine'S eingesenkten Messerchens nach Paufique wird die Ausschne idung des durch die konzentrischen Inzisionen begrenzten Ringes aus dem Corneagewebe vorgenommen und Iwar in einer gleichmaessigen Tiefe im gesamten Corneagewebe-Perimeter. (Fig. 2-B) . Mit· tels des Verschorfers von Kuhnt enHernen wir den Ienhalen Teil. wobei wir uns immer in der gleichen Ebene halten . Ohne die' Lage bezueglich des Stund en-Meridians zu veraendern und nach vorhergegangener Parazentese de¡r vorderen Kammer, wird dann die Naht am Rande vorgenommen mit 12-16 losen corneacor nea len Naehten, die gleichmae'SSig ueber den Perimeter verteilt werden. (Fi g. 2-0 ).
Bei kleineren Korrektionen ist es nicht notwendig die Cornea in ihrer Gesamtheit IU loesen, der optische Mittelteil kann hierbei be~ ,Ia uen werden. - Dieses-Vorge hen scheint Iunaechst Vorteile gegenueber einer totalen Loesung IU besitzen, jedoeh haben sieh mir bei lenterer keine wahrnehmbaren Truebungen gezeigt und ausserdem bietet diese den Vorteil, dan man keine konzentrischen Trepanen benoetigt, we lche heute schwer zu beschaffen sind.
Um diesen Abschnitt der Operation zu vereinfachen, habe ieh eine Fraese konstruieren lassen, we lche unmittelbar e ine Fa lz van der notwendigen Breite schneiden kann, WQS nieht nur weitgehend die Ausfuehrung dieser Operatio n erleichtern wird, sonde rn aueh deren Genauigkeit erhoehen wird .
Die Paraze ntese der vorderen Kammer ist angebracht, um einen allzugrossen Zug an den Nahtpunkten I U vermeiden_ Bei groesseren Korre ktione n kann die Ausfuehrung eines tempora len Elliot an ge brae ht sein .
Der postoperative Ve rla uf ist e infaeh, da
im AlIgemeinen die Anwendung von Midriatiea nieht erforderlieh ist. Die Naehte werden am 10. T'Og. entfernt. An den folgenden Tagen wird mit dem Ophtolmometer die Kruemmung der Corneo festgestellt. 1st diese zu gro'5s, so kann sie dureh Druekverhond verkleinert werden, wohei das ondere Auge freibleiht .
Um die T eehnik, mittels weleher ieh in den orstcn Faellen den Eingriff vorgenomm~n habe, zu vereinfachen, versuche ieh augenhlieklich eín neues 'Vorgehen und zwar folgendermossen :
An dem zu hehondelnden Auge wird mittels eines 11 mm. Trepans ein loger von Ih _ ~ der Dieke der Cornea ausgesehnitten, wohei, wie bei der totalen Oherflaeehen-keratopla$' m, naehfolgend die vorderen Sehichten der Cornea extirpíert werden. Dos ~o erholtene Transplantat wird in eínen konvexen und der Kruemmung der Cornea onhprecJ,enden Ausguss getan.
Mittels eíner besonderen Fixotion hleiht die Corneo an ihrem Plah und mit einem Trepan van ebenfol1s 1/ mm. wird die oben erwaehnte periphere Zone extirpiert.
Dieses Vorgehen hot den gronen Vorteil, doss es keine grosse Anzohl von verschiedenen konzenfri"Sehen Trepanen benoetigt und dass es dadureh den Eingriff vereinfaeht, do noemlieh dessen delikateste Phase, - die Extirpation des peripheren Carnea-Rings, - ausserhalh des empfangenden Auges vorgenommen werden kann • . Das Instrumentarium braucht hierbei nur aus aíner Serie von Ausguessen zu bestehen, die den verschiedenen kruemmungen der Corneen angpasd sind . Dieses Instrumentarium hat ausserdem den Vodeil unendliche Mole henutzt werden zu koennen, ohne reparaturheduerftig zu werden.
Diese T echnik hat ueberdies den Vorte.il, die mothematischen Berechnungen ueherfluessig zu machen, da die proportionaln Ziffern des Ophtalmometers ausreichen.
KORREKTlON DER HYPERMETROPIEN
Um den Kruemmungsradiu5 bei geringfueg iger Ametropíe zu vetgroessern. kann teehniseh so vorgegangen werden, wie bei den Myopien. jedoeh im umgekehrten Sínne. dass heisst, durch Hinzufuegen eines schmalen corneo len Ringes, homeoplastíseh und in der gesamten Ausdehnung des Perimeter.; der Cornea.
Dieser Eingriff erseheint schwierig und deshalb hahe ich ihn noch nieht ousgefuehrt,
obwohl er techniseh das Bede zu seín scheint. Mit Teil-Keratopladik, genauer van 6 mm ..
ist es ttloeglich ziettllich hohe Beriehtigungen zu erreichen (bis zu 10 Dioptríen), indom man die Cornea des Empfaengers bis sehr na he an die Descemet'sche Memhron oussehneidet (Fig. 3~ A-B) und sie dureh ein Transplantot von gleichmaessiger Díde erseht, welches einen Durehmeser von einígen mm./IO mehr als das Loger hat IFíg. 3-C). Die Bereehnung ist diesel be wíe im vorhergehenden Folle, jedoeh muss der Wert (m) mit 3 an Stelle von 5'5 ongenommen werden. I "m" ist die Haelfte des Durchmessers des Trepans).
Diese Methode hat mir in der Praxis die hesten Ergebnisse gebrocht. Wegen der Moeglichkeit des Auftretens leiehter, hypertrophisch-postoperativer T ruehungen, erscheint es níeht angebracht, hottleoplastische Transplantate van groeneren Ausmassen zu verwenden. Ausserdem wuerden sie eine groessere Dehnung des Descemet'schen Membran und der fiefen Schichten der Cornea erfordern, wohingegen groessere Korrektionen van mehr 015 10 Diophien, in Anhehacht der Díde der Cornea, nur mit einem verminderten Durchmesser erreicht werden koennen.
Augenblicklieh versuche ich zwei Methoden, die mehr als wahrscheinlich eine groessere Annaeherung an die diopfrische Beriehtigung ermoeglieflen werden.
Die Ente bedeht darin, ein konvexes loger mit Hilfe einer Spezialfraese auszuscJ,neiden" welches genau die in ¡edem Folle notwendigo Kruemmung hat (Fig. 4).
Ein seheibenfoermiges Transplantat van gleichmaessiger Dieke bededt es (Fig. 4-C).
Die zweite besteht darin, ein normales Lager auszusehneiden, wie bei einer gewoehnliehen laminaren Kerotoplilstik (Fig. 5~A~B" und zwisehen diesem und d~m Transplantat eíne linse aus plostischem Material einzusehen I Fíg. 5-C 1, damit diese die erwuensehte Kruemmung der Vorderflaeche der Cornea aufrechterhaelt.
Auserdem benuehen wir ihren Brechungsindex, der, um guenstig zu sein, hoch se in muesste.
Die so erhaltenen Corneen oehneln den Zeis-s'schen Kontakt-Glaesern fuer Hypermetrope, das heisst. sie weisen eínen kleínen Vorsprung von 6 mm. Durchmesser auf. waehrend der Kruemmungsradius kleiner ist 015 in der uebrigen Cornea.
