DEFINICIÓN.
INTOXICACIÓNReacción del organismo a la entrada de cualquier sustancia tóxica que causa lesión o enfermedad y en ocasiones la muerte.
El grado de toxicidad varía según la edad, sexo, estado nutricional, vías de penetración y concentración del tóxico.

Es cualquier sustancia sólida, líquida o gaseosa que en una concentración determinada puede dañar a los seres vivos.

EPIDEMIOLOGÍA.
Debajo de los 5 años de edad: • Accidentales.• – Habitualmente en el hogar.• – De consulta casi-inmediata. • – Los niños suelen estar asintomáticos.• – El tóxico es conocido.

EPIDEMIOLOGÍA.
Adolescentes, cuyas intoxicaciones se distinguen por:• Ser intencionales. • Muchas veces, fuera del hogar.• Consultar con tiempo de evolución más
prolongado.• Generan síntomas con mucha frecuencia.

Los tóxicos varían en función de la edad y del tipo de intoxicación.

USO DE LAVADO GI
INDICACIONES:Antagonitas del Ca, Fe, Litio, alcoholes, sustancias que puedan retrasar vaciado gástrico.
PROCEDIMIENTO/ DOSIS:Trendelemburg, decúbito lateral izquierdo, SNG mayor calibre.50-100 ml SS niños pequeños150-200 ml adolescentes. Extraer líquido hasta que sea claro.

• Analgésico y antipirético de uso universal particularmente en la edad pediátrica.
• 1ª causa de intoxicación medicamentosa y de insuficiencia hepática aguda.

INTOXICACIÓN POR ACETAMINOFEN
• DT: 10-15 mg/kg/dosis (máx. 1g/d, 90 mg/kg/día ó 4 g/día).• DOSIS POTENCIALMENTE TÓXICA:
Niño: 140 mg/kg.Adolescente y adulto: 8 g.

CUADRO CLÍNICO.
FASE SINTOMATOLOGÍA ANALÍTICA.
I. 0 a 24 horas. Anorexia, nauseas, vómitos, malestar general, palidez, diaforesis a partir de las 6 horas de ingesta.
Normal.
II. 24 a 48 hrs. Desaparecen los síntomas anteriores y se refiere asintomático o refiere dolor a la palpación en hipocondrio derecho.
Hipertransaminemia, QUICK alargado, hiperbilirrubinemia leve, elevación de creatinina.

CUADRO CLÍNICO.
FASE. SINTOMATOLOGÍA. ANALÍTICA.
III. 48 a 96 hrs. Anorexia, malestar, náuseas y vómitos progresivos; inicio franco de síntomas de IH y/o renal: coagulopatía, ictericia, alteraciones del nivel de conciencia u oligoanuria.
Picos analíticos de alteración hepática y renal; característcamente gran hipertransaminemia (hasta > 20,000 UI/L)
IV. 4 días a 2 semanas.
Evolución progresiva hacia el coma hepático y/o renal y exitus o autorresolución de la sintomatología.
Alteraciones progresivas o normalización en semanas.

N-ACETILCISTEÍNA.
INDICACIONES.1. Ingesta aguda (< 8 hrs). Nomograma de
Rumack-Matthew, línea de posible hepatotoxicidad.
2. Ingesta aguda > 8 hrs a dosis tóxicas.3. Ingesta aguda > 24 hrs, con detección de PCT
en sangre o signos clínicos o analíticos de afectación hepática.
4. Ingesta aguda y desconocimiento de las horas y niveles séricos PCT > 50µg/ml.

Ácido inorgánico: compuesto de hidrógeno y uno o más elementos que, cuando se disuelve en agua u otro disolvente, disocia, produciendo iones hidrógeno.
Los pH menor a 4 son los capaces de producir daños.

TRATAMIENTO.
BIPERIDENO.Anticolinérgico de acción preferente sobre el sistema nervioso central, con efectos vegetativos periféricos más débiles que la atropina.
Dosis IV: 0,04-0,1 mg/kg lenta. Se puede repetir la dosis a los 30 minutos.
