Es segura, no selecciona resistencias, no incrementa conductas de riesgo, duradera (144 semanas) y es coste-efectiva.

Tasas muy elevadas de respuesta virol para RAL QD y BID +TDF/FTC,
pero QD es inferior (el DMC suspende el estudio).
QD inferior especialmente en CV >100.000 c/mL, CD4 < 200 cels/µL y niveles bajos de RAL.
Los fracasos seleccionan frecuentemente resistencia a IN.
Los niveles bajos de RAL si se correlacionan con respuesta... y en naïves!.
En VIH debe comprobarse en ensayos aleatorizados todo lo que “aparentemente” debería funcionar bien, especialmente en situaciones
donde ya existen opciones muy seguras

Tasas muy elevadas de respuesta virol precoz a 4 (70% vs 5%) y 12
semanas (68% vs 14%) con TVR + PegIFN/RBV en genotipo 1.
Efectos adversos frecuentes (náuseas, vómitos, prurito, rash) aunque no limitantes de tto.
Tanto BOC como TVR son el equivalente a IP anti-VIH de 1ª gen: dosificación TID, interacciones PK, mala potenciación con /r, toxicidad,…
pero van a curar (RVS) muchas hepatitis C antes de ser sustituidos por
IP de 2ª gen.

Es técnicamente posible modificar linfocitos CD4 autólogos para hacerlos inmunes al VIH.
Esto se consigue por disrupción del gen CCR5 que elimina la expresión del receptor, mediante nucleasas de dedos de zinc (ZFN) y infección
celular ex vivo por vector de adenovirus deficiente que expresa las ZFN.
Consigue un incremento mantenido de CD4 en 5/6 sujetos y normaliza el
cociente CD4/CD8 en 3/5.
Parece seguro y relativamente duradero.
Estudios fase 1/2 en marcha en sujetos naïve virémicos (SB-728-1002).
