Clinical component
Case Study
When she learns that a new prison is being built near her community, Janeen is one of
the first to apply to be a nurse administrator. Janeen has a certain empathy for those in
prison, having accompanied her dad on occasion, a math teacher who volunteered his
time to teach high school and early college-level courses at a men’s prison in the state
where she grew up. She got to know a few of the inmates who valued her dad’s
contributions.
“I think having had positive experiences with inmates and gaining a certain amount of
empathy will enable me to quickly become acclimated,” she writes on her application,
“and comfortable in this work environment.”
One of the reasons Janeen would like this position is that “I have a pretty good idea of
what kinds of rules should be in place for nursing staff, which services are essential,
which should not be done by nurses, and more. Visiting the prison with my dad,” she
says, “really opened my eyes. I saw nurses doing things I didn’t think—and still don’t
think—nurses should do. Getting this job would mean I’d be in on the planning stage
NURS 403 COMMUNITY HEALTH NURSING 41
right from the start. I’d like to ensure that we minimize genuine ethical dilemmas for our
nursing staff as much as possible.”
The good news is Janeen gets the job!
On her first day at work, she is invited to a planning meeting where, among other
standard nursing responsibilities, documentation indicates that nursing staff will take
care of the following:
• Body cavity searches
• Collecting forensic data
• Force feeding during hunger strikes
“I think we need to talk about this list,” she says, despite several frowns she gets from
the correctional officers at the table.
“It’s such a short list,” says Officer Deene, who will be corrections officers supervisor of
the new institution. “What’s the problem, Janeen?”
“Well,” Janeen says, “some of these seem to me to go against the principles and code of
ethics for nurses in general. For instance, a body cavity search does not benefit the
patient’s health. Second, the nurse would be acting out of her designated professional
relationship with the patient—that of a health care giver. Third, the procedure even has
the potential to cause harm.”
“I understand that, but that’s why we’ve assigned the task to nurses,” Deene says.
“How so?” says Janeen.
Deene shrugs. “Because it does have the potential to cause harm—who would you rather
have performing it? Me or a professional nurse? With all due respect, because it has to
be done by someone, which is the lesser of two evils here?”
NURS 403 COMMUNITY HEALTH NURSING 42
Answer the following questions:
1. Which basic principle or principles might Janeen use to illustrate why nurses may
not feel ethically comfortable participating in collecting forensic data from an
inmate’s person?
2. Is it always wrong for a nurse to participate in a cavity search? How would you
respond to Officer Deene’s challenging question, in which a nurse’s participation in
the search might seem to be the “lesser of two evils” and possibly prevent harm?
3. How do you predict Janeen will settle the issue of forced feedings during a hunger
strike?
Write your answers in a Word document and upload a copy to Blackboard, in the area
designated by the facilitator.

class in a no less than three-minutes presentation. Be specific on the explained
concept and give a personal conclusion based on evidence.
3. The facilitator will gather students in groups of three students each. Each group
member will take their individual graphic organizers prepared as part of the activities
before the workshop, and create one group graphic organizer, incorporating each
member’s propositions. Each group will present to the class the graphic organizer on
the potential implications of persons knowing their genetic information on clients,
families, and communities. While presenting your information, be sure to include
ethical concepts as well as your professional opinion. Be ready to discuss questions
from the peers. The facilitator will evaluate this activity using the Oral
Communication Skills Rubric (Appendix E). This rubric can be accessed in the
Course Menu section of Blackboard under the link titled Appendices: Language
Rubrics.
4. In a question and answers game, and guided by the facilitator, each student will ask a
peer a question related to the natural history of a disease. The student chosen to
answer the question will then link his or her answer to another question for another
student, but this time, the chosen student will answer in relation to the levels of
prevention. Eventually, that student will also link his or her answer to a question on
the design and implementation of community interventions. All students will have
the opportunity to participate in this activity. The facilitator will evaluate this
activity using the Participation Rubric (Appendix F). This rubric can be accessed in
the Course Menu section of Blackboard under the link titled Appendices: Language
Rubrics.

Case Study
Elizabeta has been invited to join a team of public health nurses who have been
commissioned to plan a series on anger management for teens at three local high schools. On
her first day, she is paired with Peter, and together, the two of them plan to visit Riverview
High School to interview a group of students about what they would most like to see in a
workshop of this kind, which will be part of a 3-day fair during which students take their pick
of three workshops. Students will be required to choose one workshop on creativity, one on
social skills, and one on community outreach. In preparation for the 3-day fair, students were
asked to list topics under each of the three categories, and under “Social Skills,” the topic of
anger management was fairly in demand.
“That gets us off to a great start!” Peter says.
“I agree!” Elizabeta nods. “It’s really encouraging.”
“I think we should start by helping students understand why anger management is important.
We want to get them motivated.”
NURS 403 COMMUNITY HEALTH NURSING 78
“I’m not sure about that,” Elizabeta says. “If they’re choosing this workshop as an elective, if
some of them came up with the idea, aren’t they already motivated?”
“Well, where do you think we should start?” Peter says (slightly annoyed, but trying not to
be).
“How about a quiz?”
“A quiz? We haven’t taught them anything yet.”
“Yeah, I know, but I’m thinking of a kind of fun quiz, you know, like in a magazine.
Something like, ‘What Should You Do If . . .’ kind of quiz.”
“Wait a minute,” Peter says. “That sounds like a lot more fun, but it sorts of occurs to me that
we’re not supposed to be teaching yet. This meeting is to find out what they want and need
from the workshop.”
“Yes,” Elizabeth says, “but don’t you think a fun quiz like this will get them talking? Maybe
get them thinking about what they really do need or want instead of just asking them to talk
cold-turkey, or out of the blue, you know?”
“Maybe,” Peter says. “Yeah. Maybe you’re right. We could make some of the questions sort
of funny!”
“Yeah, then ease into more real, more serious situations.”
“Yes! Let’s do it!” Peter opens his laptop. “Okay! Let’s get started.”
1. In the area of health learning, adolescents are similar to adult learners, especially when it
comes to what motivates them. Recall four conditions listed in the text from the Knowles
and colleagues study (2005) that help motivate adults to learn and apply each to what
Elizabeta and Peter are trying to do with their quiz.
NURS 403 COMMUNITY HEALTH NURSING 79
2. “You know,” Peter says, “it occurs to me that, in a very concrete way, our quiz will be
kind of like conducting the motivational interview!” Do you agree with Peter? Explain
your rationale.
3. What are the four essential steps in MI and how can Elizabeta and Peter apply these
later—in the actual workshop itself (and not in this initial assessment and planning
meeting)?
4. Post your answers in the area designated by the facilitator in Blackboard.

multiple choice questions and students will have a maximum of 75 minutes to
complete the test.
5. Each student will deliver the presentation on the community assessment for the
specific zip code the student lives at. The presentation must include the statistics
for the specific zip code chosen, as well as the rationale for the specific assessment
method. The time allotted for the presentation is five minutes per student.
6. Using the previously created script and in a group activity, dramatize an explain
the concepts that are fundamental to community nursing practice: community,
community client, community health, and partnership for health. The
representations will be recorded (video) and uploaded to the Blackboard platform
by the end of the workshop. The facilitator will evaluate this activity using the
Role-playing Rubric (Appendix H). This rubric can be accessed in the Course
Menu section of Blackboard under the link titled Appendices: Language Rubrics.
7. Share the hardcopy of the brochure previously created with a classmate. Every
student will present his/her peer’s brochure to the class and the authors of the
brochure will expand on the concepts if needed.
Clinical component
Identify at least three organizations in your community that sponsor health
promotion and disease prevention programs. For each organization, provide
NURS 403 COMMUNITY HEALTH NURSING 96
information on the type of program, the type of interventions, eligibility for
services, and costs of services. Determine whether the program activities focus on
self-care. If possible, interview the director of one of the programs to determine the
effectiveness of these activities in your community. Write a 750-word APA style
report on your findings and upload it to the area designated by the facilitator in
Blackboard.

5. Using the previously created audiovisual presentation, deliver the revised content to
the group, be sure to include the explanation of the application of the nursing
process (assessment, planning, implementation, evaluation) to reducing family
health risks and promoting family health. Think you are delivering the presentation
to a group of community volunteers. Time allowed for the presentation per student
is 8 minutes.
6. Students will write a self-reflection describing family demographic trends and
demographic changes that affect the health of families on what they believe their
role is in epidemiologic surveillance and primary, secondary, and tertiary prevention
as nursing professionals. If deemed necessary, the facilitator may ask students to
send this activity after Workshop Twelve, but before Workshop Thirteen (Taller
Trece).
7. The facilitator will discuss and clarify any doubts regarding the activities due prior
to the next workshop.
Clinical component
The student will develop a clinical case that will include all pertinent information
related to sociocultural factors. In addition, the client's personal information will be
related to a preventable infectious condition, and the impact of this condition on the
patient's family and community must be adverse in terms of social acceptance. The
clinical case will be presented to the facilitator for approval. Once approved, the
student will create a comprehensive care plan that will include an education plan on
additional preventive measures and available community resources.

opina la clase en realidad.
iii.
NURS 403 COMMUNITY HEALTH NURSING 138
asignado.
NURS 403 COMMUNITY HEALTH NURSING 139
