Hospital admission of patients with CAP is, one of the most difficult decision for clinicians. The PneumoniaSeverity Index (PSI), recommends home-care for risk classes I and II and a short hospital stay for class III.This study investigated in 10 Spanish Hospitals the clinical use of the PSI in determining hospital admissionand the reasons why PSI was not followed.MethodsConsecutive CAP patients attending to the emergency departments during 2002 were included. PSI wascalculated by pulmonologysts who decided the treatment place.Results232 patients were included. 64% were men and 36% women. Mean age was 59.7±19.9 years. The mainreasons justifying admission in patients with class I-III were respiratory failure (43%) and clinical judgment(22%).Conclusions74% of patients with PSI I-III were hospitalized. Respiratory failure was the most frecuent reason to justifythe admission to the hospital. Backgound CAP is a common disorder with great variability in clinical practice. The decision regarding theappropiate site of care is the most important for the level of treatment and costs. Recently a hospital in our regiondeveloped a prediction rule based on the Pneumonia Severity Index (PSI) plus six additional criteria (hipoxemia<60,shock, previous correct treatment failure, social problem or inability to mantein oral intake, pleural effusion orunstable comorbidities) with an easy computer program to classify patients to be hospitalized or not (España et al.Eur Respir J 2003;21:695-701*)Objective Validate that computer program in emergency department of our hospital.Results We included between December-02 - December-04, 663 consecutive patients with CAP admitted to ouremergency department, 58 had a different final diagnosis. 285 (47%) were treated on outpatient basis. Readmissionrate was 0,06%. There was no mortality in this group.320 (53%) patients were hospitalized, 98 were PSI low riskpatients (I-II), 62 of them were admitted to hospital because of the additional criteria. 45% (28) of this ²low riskpatients² had significant complications. These resuls are similar to those obtained in Galdakao*Conclusions The application of this computer risk-stratifying program to assess admission to hospital in CAP issimple, useful, secure and can be export to different settings.
