Contexto clínico
• Paciente anticoagulado por:
– Prótesis valvular mecánica
– FA con estenosis mitral significativa
– FA con Chads-vasc ≥ 1ó2
– Otros motivos (TEP)
Ingresa por un síndrome coronario agudo

Introducción
� Los anticoagulantes orales son necesarios en pacientes con
prótesis mecánicas, TEP o FA con CHA2DS2-VASC ≥ 1-2
� 30% de estos pacientes tiene además enfermedad coronaria
� La doble antiagregación plaquetaria (DAP) es necesaria para la
prevención de la trombosis del stent
� La triple terapia (TT) implica un riesgo alto de sangrado anual
(hasta 45%)*
� La incidencia de sangrado mayor se relaciona con la duración de
la triple terapia (4.6% vs. 10.3% con TT 1 mes vs. 6-12 meses)

� La retirada precoz de la doble antiagregación es un
factor de riesgo independiente de TS
� La mayor parte de los casos de TS se manifiestan
como SCACEST

Results
Conclusions• Among patients with nonvalvular AF who underwent
PCI, a rivaroxaban-based strategy was associated with a lower frequency of clinically significant bleeding compared with a warfarin/DAPT strategy
• Stent thrombosis appeared to be similar between the three groups

Conclusions
1. First randomized trial to address the optimal antiplatelet therapy in patients on OAC undergoing coronary stenting
2. In this study which was specifically designed to detect bleeding events, the bleeding rate was higher than expected
3. Primary endpoint was met: OAC plus clopidogrel causes less bleeding than triple antithrombotic therapy, but now shown in a randomized way
4. Secondary endpoint was met: with double therapy there is no excess of thrombotic/thromboembolic events: stroke, stent thrombosis, target vessel revascularisation, myocardial infarction or death
5. Less all-cause mortality with double therapy
