PERITONEAL DIALYSIS IN PATIENTS WITH VENTRICULOPERITONEAL SHUNT Restrepo Jaime, Maria Zapata, Patricia Hernandez, R. Gomez-Acevedo, Alexander Lovera RTS Versailles, Cali, Colombia Introduction Peritoneal dialysis is the therapy of choice in both pediatric and adult patients for their safety, effectiveness and comfort. There is a group of patients with chronic renal failure with a history of ventriculo peritoneal and spina bifida in whom this therapy contraindicated literature and prefer to leave these patients on hemodialysis. Target We want to present work to show our experience of patients with VP shunt for different reasons such as difficulties in vascular access or social conditions could not leave them on hemodialysis therapy and were placed a peritoneal catheter. Materials and methods Since 2003 to date we have had 4 peritoneal dialysis patients with ventriculo peritoneal, 2 children and 2 adults, 3 and 1 in CAPD APD, 2 patients with lumbosacral myelomeningocele diagnosis, 1 with encephalocele and the other patient (adult ) with hydrocephalus secondary to head trauma, 4 patients had their ventriculo peritoneal running when starting peritoneal dialysis therapy. The ages of the patients at initiation of therapy were 6, 10, 17 and 80 years, 2 patients are now transplanted and the other 2 patients still continue on peritoneal dialysis therapy with a stay at 60, 9, 23 and 15 months respectively. The 4 patients therapy was not performed by the patient but by a primary caregiver trained in the renal unit, the stratum socioecomico was 2 and 3 (range 1 to 6, 6 being the upper stratum), with suitable environmental conditions in 3 of them, 1 patient had to make adjustments in the spare room as traditional sink and cover with plastic walls.
Results One patient had an episode of peritonitis at 6 months after initiation of therapy for catheter rupture that was diagnosed by abdominal pain and cloudy fluid, is taken to surgery where he removed the catheter and placed tenckoff temporarily in hemodialysis 3 months, did not present any neurological complication and again placed peritoneal dialysis catheter. None of the patients presented ultrafiltration failure being the same on average 1 to 1.5 cc / kg / hour, all patients achieved goals kt / v greater than 1.7. Conclusions This description of cases of patients with peritoneal shunts PD has been a safe therapeutic option and have reached levels of efficacy of therapy. Time in therapy of these patients has been more than 9 months. In the cases presented have required 100% support of a trained caregiver for therapy. We recommend a review of more similar clinical case series to determine the specific care recommendations of this group of patients where DP is a viable alternative
