CARCINOMA DE MERKEL
• Podemos considerar al carcinoma de Merkel como una enfermedad rara.
– 0,2-0,8 casos/100,000 habitantes
– 33% de mortalidad (en comparación con melanoma, sería el triple!)
• Y de descripción relativamente reciente (1972)
– Toker: Trabecular Carcinoma of the Skin (Arch Dermatol 1972)
– Debido a esto, quizá aumenten los diagnósticos por: • Envejecimiento de la población
• Mayor supervivencia de inmunosuprimidos
• El conocimiento de la enfermedad

LA IMPORTANCIA DE LA ESTADIFICACIÓN Y TRATAMIENTO LOCORREGIONAL
• Cirugía primario +/-RDT único tratamiento curativo – “Filosofía” melanoma:
• 1-2 cm margen
• ganglio centinela en ganglios clínica y radiológicamente negativos
• Linfadenectomía en ganglios positivos clínicos o tras BSGC positiva
• Radioterapia adyuvante: fundamental en estadio I/II, aumenta SG – Ganglios clínicamente negativos:
• Zona tumoral a todos, salvo < 1cm y sin invasion linfovascular
• Lecho ganglionar: Ganglio centinela positivo y no linfadenectomía
• Ganglio centinela no posible o negative con riesgo de falso negativo (por ej, cabeza y cuello)
– Papel muy discutido tras linfadenectomía, salvo multiples ganglios y/o afectación extracapsular (y aún así, no impacto SG)
• No hay papel estándar para la QT adyuvante

RECAÍDA Y ENFERMEDAD METASTÁSICA, ¿ALGUNA NOVEDAD?
• Enfermedad quimiosensible pero rápidamente quimiorresistente – “Filosofía CPCP”
– Regímenes basados en Platino/etopósido, alquilantes, taxanos…
• No basados en estudios aleatorizados
• No pretendamos tampoco tenerlos
– Segunda línea = quimera
• Racional para inmunoterapia – Mayor frecuencia en inmunodeprimidos
– Casos de regression espontánea
– Infiltración linfocitaria = mejor pronóstico

• Anticuerpo monoclonal totalmente humano IgG1 que inhibe PD-L1; cierta actividad ADCC

• Tumor cutáneo muy agresivo y raro, que afecta a personas mayores en zona fotoexpuestas
• Es esperable aumento de incidencia por – Envejecimiento
– Mejoras diagnósticas
– Se va a afinar más el diagnóstico diferencial entre CPCP y MC
• Tratamiento locorregional complejo = equipos multidisciplinares – Cirugía, ganglio centinela, linfadenectomía, RDT
– QT adyuvante no es estándar
• Es más que probable que la inmunoterapia basada en anti PD-1-anti PD-L1 sea la opción estándar en breve
