• 1. En pacientes sin contraindicacion medica � CIR
• 2. EI no quirurgicos (comorbilidad local ó sistémica, ó negativa a la CIR) � RDT externa TC (60-70 Gy)
• 3. Valoracion de terapias alternativas, ablacion percutanea (RDF) ó Radioterapia estereotactica extracraneal (SBRT)en Tumores Perifericos
• SBRT Alternativa válida a la Lobectomia en E I
- Local control 90% a 5 a. (actuarial)
- Supervivencia 60 - 80% a 5 a. (actuarial)�
- Toxicidades Grado III- IV <15 -20%
• Menos invasiva que la toracotomia
• Menos costosa
• Proliferan los estudios

Pacientes con buen PS y tumores pequeños, tratados con RDT a Dosis � 65 Gy. Aumentaba la SLE (posibilidad real de larga supervivencia y/ó curación).

Revision de 10 estudios de pacientes medicamente inoperables (T1 T2, No) tratados con RDT exclusiva a 60-66 Gy. SG a 5 a. 15%, pero 25% muerte por enfermedad intercurrente, 30% por enfermedad metastasica , y solo el 30% presentan recaida local exclusiva. Toxicidad G 3-5 <2% .Sibley GS . Radiation Oncology, Duke University Medical Center, Durham, North Carolina 27516, USA. (Cancer 1998).
2000 paciente irresecables / 26 estudios retrospectivos no randomizados de “RDT RADICAL” (DT>40Gy)
SG : 33%-72% a 2 a., 17%-55% a 3 a. y 0%-42% (30%) a 5 a.CHART vs 60Gy/6s 37% vs 24% a 2 a.
SEC: 54%-93% a 2 a., 22%-56% a 3 a. y 13%-39% a 5
RC 33% (60 Gy) - 61% (72Gy)
Recaida L 6%-70%. M1 25% de los pacientes.
Las mejores tasas de respuesta y supervivencia eran en los tumores más pequeños y las dosis más altas de RDT

• Destacando que después de la radioterapia entre el 11 y el 43% de los paciente murieron por causas no relacionadas con la neoplasia, denotando el MAL ESTADO DE SALUD DE DICHOS PACIENTES.

Los falsos negativos están disminuyendo gracias a una mejora en sensibilidad en el PET-TC: ↑ sensibilidad y especificidad (83% y 92%) respecto al TC

Estudios de Escalada de dosis con radioterapia 3D, técnicas de conformación y dosis/f 2.15 Gy
Dosis de 83.8 Gy en pacientes V(20) <25% Dosis de 77.4 Gy en pacientes V(20) entre 25% y 36%Dosis superiores de hasta 90.3 Gy provocaban niveles no
aceptables de toxicidad ( 2 muertes toxicas).
Recaida ganglionar fuera del campo de irradiación <10% de los pacientes.

STEREOTACTIC BODY RADIATION THERAPY (SBRT) en CPNCP
• Stereotactic Radiosurgery (SRS) administra muy altas dosis de irradiacion por sesión, a pequeño volumenes, con muy alta precisión
• Introducida en 1960 en el tratamiento de tumores primarios o metastásicos cerebrales.
• Adaptada al tratamiento de localizacionesextracraneales como pulmón, higado, lesiones paravertebrales (SBRT)
